Dear Parent/Guardian,

 

We are currently accepting applications for 2013-2014.  Your child must be 3 or 4 years of age before September 1, 2013 to be eligible for the upcoming school year.  Recruitment for Head Start/Pre-K is an ongoing process.  Emphasis is put forth in the spring and summer months for the upcoming year.  Each family interested in Head Start/Pre-K will be asked to complete a universal application for the program.  Although this does not guarantee enrollment, it does begin the process.  There are two mandatory requirements, but three actual requirements that must be met before your child can actually attend class.

 

State certified birth certificate*.  This must be obtained from the State Division of Vital Statistics.  We can no longer accept hospital birth certificates.  State law requires the certified certificate prior to entry into a school or Head Start setting.  You must produce evidence of the state certified birth certificate at the time of enrollment.  The Family Service Staff can give you the correct forms and guidance on how to obtain this document if you have not done so already.  (Must bring a copy at time of enrollment)

 

Income verification.  At the time of enrollment, Head Start Staff must have verification of the previous twelve months of income, if the child is not a returning child to the program.  Even if you have “no” income, (perhaps you are living with a relative or friend), you must have a note or letter from the relative or friend that you are in fact living with them and they are supporting you in terms of shelter, food, clothing, etc. All families must be within the income guidelines to be enrolled into the program, or meet other criteria as set forth by the agency.  There will be no exceptions.  Family Service staff will have all of the information to guide you through this process. (Must bring a copy at time of enrollment)

 

Immunizations. Your child must have all immunizations up-to-date as per regulations of the Center for Disease Control and the State of WV.  The Family Service Worker, your family physician, or the local health department will have the lists that are required. (This includes the Hepatitis A booster)  Head Start staff will assist you in any way we can so that your child meets this regulation.  For required physicals, we must meet the WV Health Check form. (Must bring a copy at time of enrollment)

 

When you are actually enrolled into the program, whether it is in a school setting or one of our Head Start sites, you will have thirty (30) days to obtain the above documentation.  If you fail to do so, then we will have no choice but to place your child on our waiting list until all necessary requirements are met.  We are here to help you and meet the needs of your family in every way that we can.  Please do no hesitate to ask for help.  We will do everything that we can to do so.

 

*If your child was not born in WV, we will need a certified birth certificate from the state in which he/she was born.  The same applies with immunizations.
